Materials and methods

Between January 1991 and December 1997, a total of 411 living-donor renal transplants were performed in Hanyang University Hospital Transplantation Unit. Of these, 161 patients received grafts from living-related donors; 61 patients received grafts from exchange donors; and 189 patients received grafts from living-unrelated donors. The exchange-donor program was such that the volunteers were second-degree relatives, friends, and spouses and were matched for renal transplantations. A more

Results

Since 1991, we have used the exchange-donor program for the first time in Korea (Fig 1). Of the 61 patients who received kidneys from exchange donors, 59 are currently alive, with a mean follow-up of 30.8 months. Two patients died from accelerated acute rejection and sepsis after transplantation. Of 161 patients who received kidneys from living-related donors (LRD) 156 are alive. The mean follow-up for the living-related donor group is 45 months. The mean serum creatinine level of those

Discussion

A huge disparity in the number of donors and recipients has led to renewed interest in alternative ways to increase the donor pool. These include the use of marginal and non-heart-beating donors and living-unrelated donors. We propose an exchange-donor program that will be able to expand the donor pool and overcome the shortage of cadaveric organ donors.6, 7, 8 To select a donor, we must consider many things, such as a complete lack of any HLA and/or ABO-matched living-related donor, no
